January 30

Brenda is still in the Medical/Surgical ICU listed as critical with focus on Kidney and watching the Lungs. This morning was her dressing treatment and the process took only 45 minutes. Next, was her Respiratory treatment, Occupational therapy moved her legs / feet, and Physical therapy set her up for about 35 minutes and at that time did her exercises. Again, she did an amazing job. She really is fighting. Her burns are healing as well. Her Ventilator setting is at a PEEP of 7 and Oxygen level is at 40%. She is receiving antibiotics via the Ventilator, fourteen day cycle that will hopefully be ending Friday. Her white cell count is normal. Urine output was good and they are monitoring her process of Toxin's, but she still needs to process a much higher level. If she continues to improve her process levels of Toxin's they will not need to do the Hemodialysis. This is a day by day decision based on her kidney toxin process levels. She was in the Stretch Chair this afternoon for 3 plus hours at which time she was able to watch some TV, and Jack managed to work with her on speech exercises. While the last two days have been very uplifting we are reminded that Burn Patients have a different recovery cycle, and that means that at anytime she could have a major problem that would pause her recovery. Celebrate the little steps, but pray that big problems do not arise. [...]

January 29

 Brenda is still in the Medical/Surgical ICU listed as critical with focus on Pulmonary and Kidney. The first thing this morning was her dressing treatment, it took only 45 minutes, and then they did her Respiratory treatment, Occupational movement of legs / feet for abut 28 minutes and then Physical therapy arm movement plus they set her up for about 15 minutes. This was one of her best days since her move to the Medical/Surgical ICU. Her burns are healing. Her Ventilator setting is at a PEEP of 7 and Oxygen level is at 40%, this down from the 50% she was at. She is receiving antibiotics via the Ventilator, fourteen day cycle that should be ending Friday. Her white cell count is normal. Urine output was good and she did process an increased amount of Toxin's, but she  still needs to process a much higher level. If she continues to improve her process levels of Toxin's they will not need to do the Hemodialysis. This is a day by day decision based on her kidney toxin process levels. She was in the Stretch Chair this morning / afternoon for 3 plus hours. Jack is working with her with speech exercises which she is working hard on. Doctor Perry, her team lead, said he is going to order an increase in her physical therapy since she is doing so much better. If her pulmonary keeps improving she may move back to the Burn ICU, still critical, but the focus is on burn care.[...]

January 28

Brenda is still in the Medical/Surgical ICU listed as critical. First thing this morning, they did her Respiratory treatment and then her dressing change. Her burns are healing. Her Ventilator setting as at PEEP of 7 and Oxygen is at 50%, up from 40%. Urine output was good, but she did not process Toxin's. This means she has Acute Kidney. They treat this by doing Hemodialysis starting to today continuing with treatments Tuesday, Thursday, and Saturday. The goal is to clean the kidney and have the kidney start processing the toxins. The Physical Therapist had Brenda setting up at the side of the bed this afternoon. The plan is to have her in the Stretch Chair this afternoon.[...]

January 27th

First thing this morning they did her dressing change. Her burns are healing. The respiratory doctor changed her PEEP setting from 5 to 7 due to her struggles breathing last night. She is breathing better today. She was in the stretch chair for about 5 hours today, this helps her breathing. The renal doctor put her on dieretic to increase her urine output. They are monitoring her kidneys to make sure she is processing toxins. No dialysis today.[...]

January 26th

Brenda is still critical in medical/surgical ICU. The respiratory therapist has been trying towean her off of the ventilator. They tried the Cpack setting, so she was breathing in her own. She did this for a short period of time. The renial doctor reported that her kidney output was acceptable. However, she is not processing toxins so they are going to put her on hemoglobin dialysis today. Her heart rate is being controlled by medication, this will be done until they can get the Afib under control.[...]

January 25th

Today, Brenda remains in the Surgical/Medical ICU on the Triadyne bed listed as critical. All her numbers remained stable. She is on a diuretic and off dialysis. She sat up in the stretch chair for an hour and 45 minutes. She did her first physical therapy, it was just the beginning steps. The speech therapist came in and worked with her today and her cultures are negative.[...]

January 24th

Brenda is still in the Surgical/Medical ICU on the Triadyne bed, she is listed as critical. Her Ventilator number improved again and she has new setting, PEEP is now 5 and Oxygen remains at 35. Today the X-rays of her lungs show improvement. They changed her dressing this morning. At noon they stopped her Dialysis. The Renial doctor will be watching her kidney output. Her Hemoglobin was low, she received one unit of blood. Her white count went up today and they are taking a culture to determine the source. This afternoon her blood pressure went down and they gave her fluids to improve it. Some progress with lots of concerns about blood pressure. The doctors are watching this very closely. This certainly is a long trip with many highs and many lows.[...]

January 23rd

Brenda is still in the Surgical/Medical ICU today. Her Ventilator number improved again and she has new setting,  her PEP is now 8. The X-rays of her lungs show improvement as well. She is still on Dialysis, her rate has been increased to 200 per hour. She is tolerating this setting; heart rate and blood pressure are within the acceptable limits. Her burns are healing. They are scheduled to take cultures today and her results should be in tomorrow. John and Jack read to her the wonderful get well cards today. We are taking each day as a good day by accepting no regression, or a slight movement forward. We are keeping in mind that she has a long way to go.[...]

January 22nd

 Her Ventilator numbers have improved, her oxygen setting is 35 down from 45 and PEP is 12. The Xrays of her lungs show improvement from yesterday. She is still on Dialysis due the need to remove fluid, her rate of withdraw was increased from 100 to 150. She is tolerating this well, no increase in heart rate and her blood pressure remains steady. Her kidney's have not kicked in yet and they are in the process of changing her treatment plan. She is only receiving Heparin via the drip and other pain medication as necessary.They stopped the antibiotic drugs about 2 days ago and the anti fungal treatment will end tomorrow as this completes the 14th day of her treatment cycle. Her white cell count is up a little and tomorrow they will take cultures to verify there is no infection. If there is infection they will treat accordingly. Today is just one very small step in the long way home.[...]

January 21st

Brenda was moved to the Triadyne bed in the ICU, which is similiar to the RotoProne, but it only moves the upper body 5 degrees. She is still on dialysis,  because there is more fluid to remove. Her kidney numbers are showing a slight improvement. All her numbers are in the acceptable range. They have reduced the amount of medication. She remains on the ventilator and she is receiving respatory treatments and dressing changes. She still has a long ways to go, but we take every little step forward.[...]
